Where do I get the published subcontractors statement form?
- Open the claim and go to Declaration. Read what YourCA filled in. That list is what it holds from your business profile: your business name, ABN and address, who signs and their position, your exempt employer answer and your signature. Nothing in it is sworn.
- Anything YourCA does not hold says Not added yet, with Add beside it. A missing signature says Not signed yet, with Sign. What you add writes to your business profile, so it fills in on the next form and on every job.
- Only a company administrator can fix one. Anyone else gets a line that names who can change it, not a button that would be refused.
- Under that are the answers only you can give. YourCA never pre-selects one, because each is a statement you make.
- Press Download, under Your copies, for the two page NSW Subcontractor’s Statement. Its own boxes carry your business name, ABN and address, the head contractor and the contract number from the job, and the claim period. Every field stays editable. The Notes page is as published.
- The head contractor’s ABN comes from your own project directory, where you typed it against that company. YourCA does not look one up, and does not work one out from their name. If the directory has none, that box on the form is ruled and empty for you to write in.
- If YourCA cannot fill a box from your own records, the box is ruled and empty on the form. The form never prints why a box is empty, because that is about your records, not the statement.
- Answer the exempt employer question, which decides the two boxes. If you say you are one, the exempt employer box is ticked. If you say you are not, paragraph (a) is ticked.
- If you leave it unanswered, both boxes stay empty, and the row says Not answered so you see it before you download. YourCA never works this out from your records. Ask about the exempt employer question for what it means.
- Write in the date of your Certificate of Currency. YourCA holds the date a certificate is current TO, which is its expiry. To put that in a box that asks when it was dated would be a false statement about an insurance document.
- Download asks which copy you want. PDF, signed puts the signature held for your business signatory onto paragraph (f), with the name, the position and the date beside it.
- If no signature is held, the screen offers to take one. Draw it, or photograph a signed sheet of white paper and YourCA takes out the paper. It saves to your account, so every device you sign in on prints the same mark.
Worth knowing
YourCA never pre-signs a form that must be witnessed. You make a statutory declaration in front of a Justice of the Peace or a solicitor, and a stored image is not that. Print it and sign it in front of them. Signing is also always a press. No setting puts your signature on a document for you.
