Each point says what is checked, what it must meet, and who must be there.
A hold point stops the work until it is signed. A witness point means somebody may attend, and the work continues if they do not.
A point signed at the time, with a photo, is evidence. A signature added a week later is worth much less. After the wall is closed, the argument is about memory.
In YourCA, inspection points live in the Site tab, because you sign them while you stand in front of the item inspected.
An unsigned hold point shows later as a blocked claim line, with the reason on it. So the paperwork and the claim are one record, not two.
